Online Contact Form

How it works:

The user fills out the open form fields.

The information can be delivered to you in a number ways. It can      be sent to you in an email, self populating pdf file, or a text file.

The information could also be sent to multiple email addresses, or      give the user the option send to sales, marketing, support, etc.

Some fields are required like name and email address.

If the user neglects the required form fields, the system will     prompt them about the missed fields, and the form will not be     sent.
